Effective May 1, 2021, keratitis and corneal ulcers diagnosis codes (H16.001 - H16.079) will be removed from the list of payable diagnoses for necessary contact lens claims for VSP patients. As part of a standard policy review conducted by the VSP Quality Management Committee, it was determined that keratitis diagnosis codes associated with acute corneal infections or ulcers do not require contact lenses for treatment.
