Essential Medical Eye Care Punctal Plug Update

Effective February 1, 2026, VSP^® Vision Care has simplified billing of Punctal Plugs under Essential Medical Eye Care. With this update, you can: 

• File claims for Punctal Occlusion by Plug (CPT code 68761) with a maximum of six punctal plugs per patient’s lifetime.  
  • The six per lifetime is in any combination of temporary or permanent plugs. The supply for both temporary and permanent plugs is included in the procedure reimbursement amount.  
• The modifier “SC” is no longer required for billing temporary plugs.
