Updated Patient Condition Sticky Notes

To support your practice with including diabetic exam findings on your VSP claims, we updated the patient condition sticky notes to include Current Procedural Terminology (CPT) Category II Codes.

Now you can quickly mark which patient conditions and Diabetic Eye Exam CPT Category II Codes are relevant to the patient during the exam, making it easy to transfer them to the claim submission later. 

These newly updated sticky note pads are available to order by logging in to eyefinity.com, then going to VSPOnline and navigating to Administration > Supply Request Form. You’ll then complete the form including checking the box next to Patient Condition Sticky Notes.
